The 4th Annual NJ Conference on Collaborative Stroke Management will continue to address the development of collaborative partnerships between primary and comprehensive stroke centers. It will focus on both the complexities involved in maintaining collaborative relationships across a health system network, as well as with other neighboring facilities outside the network. The Stroke Council will be described as a mechanism for facilitating integration within the Hackensack Meridian Health Northern, Central and Southern regions of New Jersey. Promoting collaboration with other facilities across the State of New Jersey will also be discussed. Process improvement in stroke systems of care, including benefits for patients and facilities will be explained. Future directions in expediting delivery of care will be reviewed.

Emergency stroke management will be covered. Critical elements for stroke care optimized by the Emergency Department will be reviewed. This will be followed by a related talk on future innovations in the care of stroke patients within the emergency department setting and in concert with emergency medical services. Clinical approaches to patient imaging will be illustrated. Neurosurgical procedures appropriate for patients who have suffered strokes and other neurovascular cases will be presented. Innovative future approaches to stroke rehabilitation will be discussed, including the evidence from current research in progress. The intricate role of nursing from both the comprehensive and primary stroke center perspectives will
be discussed.

Opportunities for collaboration through education and research among physicians and other stroke clinicians across the network will be described. The conference will close with a panel of physicians discussing cases with unique stroke presentations. The panelists will represent primary and comprehensive stroke centers who will illustrate various facets of stroke care with emphasis on the manner in which comprehensive and primary stroke centers can collaborate in providing optimal care for the patient and community. Moderators will give their input and experience on different aspects of specialized care being discussed in each segment of the program.
